Wilkes County Veterinary Services Boarding Agreement

Requirements to Board:
Dogs: Must have current Rabies, DHAPP, and Kennel Cough vaccines, as well as a negative fecal within the last year. Please enter current Flea/Tick medication below (if any).
Cats: Must have current Rabies vaccine.
If these were completed at a different facility, it is the owner’s responsibility to provide records prior to the animal entering the boarding facility. If no records are provided, WCVS will give the previously mentioned vaccines and fecal check and the owner will be responsible for the cost.

By signing below, you understand that your animal must be current on the listed vaccines and you are to provide proof. All fees are required to be paid at time of pick up. Pick up is only completed during regular business hours. If your pet is found to have fleas, it will be treated at your expense. If an issue arises with your pet, the staff will attempt to contact you on the below listed number. If it is not possible to reach you in a timely matter, the staff at WCVS has the authority to make decisions in your pet’s best interest.
