1. Grooming Agreement.

By signing below, you authorize West Hills Animal Hospital to perform grooming services on your pet. You understand that while every precaution will be taken to ensure your pet's safety and well-being, grooming can sometimes cause stress to animals. You agree to provide us with accurate information regarding your pet's health and temperament to minimize any risks. You authorize the reasonable use of safe-handling tools (e.g., muzzles) if needed to protect staff and your pet.

2. Social Interaction Preference.

Please select one of the following options regarding your dog's interaction with other dogs during their grooming visit:

A. Social Interaction: I consent to my dog interacting with other friendly, vaccinated dogs during their time at the grooming facility. I understand that this may include supervised time in a common area. I acknowledge the inherent risks of dog-on-dog interaction despite reasonable supervision; West Hills may remove my dog from social areas at any time for safety.

B. Isolation: I prefer my dog to be kept separate and isolated from other dogs at all times during their grooming appointment. I understand this means my dog will not have direct contact with other pets in the waiting or grooming areas, other than brief, necessary movement by staff.

3. Health and Safety.

Vaccinations: You confirm that your pet is up to date on all required vaccinations.

Temperament: You confirm that you have disclosed any history of aggression, anxiety, or other behavioral issues your dog may have toward people or other animals.

Injury and Ailments: You authorize the grooming staff to immediately notify a veterinarian at West Hills Animal Hospital if any health concerns, injuries, or illnesses are noticed during the grooming process.

4. Emergency Care.

If urgent care is needed, grooming staff will promptly alert a West Hills veterinarian. For non-urgent issues, staff will attempt to contact me before proceeding. I remain financially responsible for any veterinary care authorized by me that is not the result of West Hills’ negligent acts or omissions.

By signing this form, you acknowledge that you have read, understood, and agreed to the terms outlined above, and that you are the owner or have authority to consent for the owner. Payment for authorized services is due at discharge. This consent remains effective for 12 months from the date signed unless revoked in writing.
