As up to 60% of a pet’s teeth are located beneath the gum line, it can be difficult to fully assess the condition of their teeth without the use of dental radiographs (X-rays). Dental X-rays are an essential diagnostic tool that can reveal conditions that may otherwise go undetected. For example, X-rays allow the veterinarian to identify fractured or damaged tooth roots, cysts, tooth resorption, early tumors, and unerupted teeth. They also provide a clearer view of teeth that may appear healthy on the surface but may be compromised internally.
Due to the limitations of visual examination alone, it is impossible to accurately predict the exact condition of each tooth or whether an extraction will be necessary prior to performing X-rays. By signing this form, you are authorizing the attending veterinarian to perform any extractions, procedures, and/or treatments deemed necessary based on their professional judgment.
Please note: If you choose not to provide consent for these procedures, the dental procedure will be canceled, and a $250 cancellation fee will be applied.
Authorization and Financial Responsibility:
I, the undersigned, certify that I have read and fully understand this authorization for treatment. I am the owner or legally authorized agent of the animal described above and have the authority to provide consent for the procedures outlined. I assume full financial responsibility for all charges incurred during the treatment of the animal and agree to pay all fees in full upon the animal's release from the hospital.
In the event of an emergency, I understand that necessary treatments will be provided at my expense, and I will be contacted as soon as possible. I acknowledge that, while every effort will be made to ensure the safety and well-being of my pet, there are inherent risks involved with any procedure, especially anesthesia, and that the outcome cannot be guaranteed.
