Connecticut Puppy/Kitten Purchasers:
I understand that I am purchasing this puppy and/or kitten with the understanding that the
associated pet store listed above provide “Free Limited Health Care” at any of their referring veterinarians. The first initial exam will be covered by the pet store within 20 days of purchase.

I have been made aware by the staff at the associated pet store that colds, coughs, and the need for de-worming are very common when purchasing a new puppy and/or kitten.

I understand that if my pet is feeling well with no concerns, only the examination and parasite treatment (if deemed necessary) is covered by said pet store. I accept full responsibility for wellness services, vaccinations, fecal testing, and further treatments as recommended by the veterinarian.

Initial examination coverage shall only apply if Court Square Animal Hospital performs the pet's first lifetime veterinary visit. Should the pet's first examination occur at any other facility, coverage for that initial examination and subsequent treatments is null and void.

I authorize Court Square Animal Hospital to release any necessary medical records to the
associated pet store if requested or deemed necessary.

If at any time after the purchase I choose to use any veterinarian out the associated pet
store’s network, I will accept full responsibility for any fees/costs associated with the visit
regardless of the diagnosis. I will not hold the associated pet store responsible for any vet bills I incur if I use a veterinarian outside of the network.

Pet Store Covered Services

The following services are covered:
• 1st exam if the pet is seen within 20 days of purchase.
• Fecal parasite treatment is covered up to two times if the fecal sample is positive.
*Please note – we recommend a fecal sample (not covered) to be brought in for this first visit.
If the pet has a pre-existing illness/injury related to the purchase of the pet (a pre-existing condition from before the date of purchase) the following are also covered:
• Medical care for the pre-purchase illness/injury
• Diagnostic BW for the pre-purchase illness/injury
• Medications for the pre-purchase illness/injury
• Recheck visits for the pre-purchase illness/injury
*Please note - these things are only covered until the pre-purchase illness/injury is resolved.
Not covered for any reason:
• Fecal Samples – we recommend owners bring this in for the first visit
• Supplements
• Prescription Diets
• Exams or Recheck visits if any services unrelated to a pre-purchase illness/injury are performed. (ex: a pet comes in to recheck cough from the time of purchase, but the cough is resolved so a vaccine is given. The owner is now responsible for this visit because we administered a vaccine).
