Emergency contact is authorized to make any treatment decisions to include medication, diagnostics, and quality of life decisions.

1.) All animals must be current on all required vaccinations. It is the responsibility of the owner to provide proof of vaccines. If no proof is provided at the time of check in the pet will receive the required vaccines with an exam at the owner's expense.- Canine Required vaccines: Rabies, DAPP or DHPP, Lepto and Bordetella.- Feline Required vaccines: Rabies, FVRCP

2.) All animals must be free of internal and external parasites, or they will be treated at the owner's expense (Fleas, Ticks, Tapeworms, etc.).
3.) Boarders may be picked up between 7:30AM and 5:30PM Monday through Friday and Saturday 7:30 - 12:30 . For the safety of our staff and patients there are NO pickups Sunday. No exceptions.
4.) Boarding charges are per pet, per night regardless of pets boarding together.
5.) If a boarder presents with ANY illness while boarding (ex: vomiting, diarrhea, anorexia, etc.), a staff member will attempt to contact owner at the number(s) provided. If contact can not be made, Belton Small Animal Clinic has permission to treat the patient per doctor discretion at the owner's expense. This may include diagnostic testing.
6.) We like to provide pets with bedding/ blankets and enrichment toy time while your pet is boarding. This includes tennis balls, Kongs, and other toys, however please let us know if your pet is a chewer. Belton Small Animal Clinic will not be responsible for illness/ obstruction due to pets chewing/ eating bedding or toys provided. Would you like us to provide bedding and toys?

7.) Feeding/ Allergies: Sometimes pets are reluctant to eat while here. Is it okay for your pet to have treats or added canned food to their diet while boarding? If your pet has any food allergies or is on a special or prescription diet please select NO.

I understand and acknowledge that the Facility is not equipped with an on-site fire suppression sprinkler system (as is allowed by Texas law). I further and acknowledge that Belton Small Animal Clinic is not staffed 24 hours (we are not staffed overnight or during staff breaks on the weekends)and that my pet will be left unattended during those times during their boarding stay. By signing this consent form, I agree to release the Facility, its owners, employees and agents from any and all liabilities, claims or expenses arising from my pet’s boarding stay, including but not limited to injuries, illnesses or death. I have read and understood the statements outlined in the notice and in this consent form and release. I voluntarily consent to my pet staying at the Facility and acknowledge that I am solely responsible for any consequences that might arise during their stay.
** Belton Small Animal Clinic is not responsible for items left, lost, or damaged while boarding.
I authorize Belton Small Animal Clinic to treat my pet in case of emergency. I understand that the staff will contact me as soon as possible and that some treatments may result in additional charges. I understand that if any estimate for service is provided to me, it is only an estimated price. I acknowledge that Belton Small Animal Clinic attempts to give as accurate of estimates as possible; however, I understand that there may be unforeseen complications; including death, and further treatment may be necessary during an emergency. If I am unable to be reached for questions at the number I provided, I understand that the veterinarians at Belton Small Animal Clinic will use their professional judgment with regards to the treatment of my pet. This may involve additional charges. By signing below, I acknowledge that I am the owner/agent of the animal in question. I accept and assume full and total financial responsibility for any and all services rendered.
I understand that payment is due when services are rendered. I have read and understand the boarding policy, requirements, and notice of lack of on-site fire suppression sprinkler system, and no overnight staffing. I agree to board my pet with Belton Small Animal Clinic releasing the facility, its owners, employees and agents from any and all liabilities.
