1. All animals must be current on all required vaccinations. It is the responsibility of the owner to provide proof of vaccines. If no proof is provided at the time of check in the pet will receive the required vaccines with an exam at the owner's expense.
- Canine Required vaccines: Rabies, DAPP or DHPP, Lepto and Bordetella.
- Feline Required vaccines: Rabies, FVRCP
2. All animals must be free of internal and external parasites, or they will be treated at the owner's expense (Fleas, Ticks,Tapeworms, etc.).
3. Boarders may be picked up between 7:30AM and 5:30PM Monday through Friday and Saturday 7:30 - 12:30 . For the safety of our staff and patients there are NO pickups Sunday. No exceptions.
4. Boarding charges are per pet, per night regardless of pets boarding together.
5. While your pet is in the care for boarding at Belton Small Animal Clinic we want to ensure we honor your wishes in the unlikely event of a life-threatening emergency.

6. If a boarder presents with ANY illness while boarding (ex: vomiting, diarrhea, anorexia, etc.), a staff member will attempt to contact owner at the number(s) provided. If contact can not be made, Belton Small Animal Clinic has permission to treat the patient per doctor discretion at the owner's expense. This may include diagnostic testing.
7. We like to provide pets with bedding/ blankets, however please let us know if your pet is a chewer. Belton Small Animal Clinic will not be responsible for illness due to pets chewing/ eating bedding provided.

8. We would enjoy enrichment time with your pet while boarding. This includes tennis balls, Kongs, and other toys, however please let us know if your pet is a chewer. Belton Small Animal Clinic will not be responsible for illness/obstruction due to pets chewing/swallowing toys.

9. Feeding/Allergies: Sometimes pets are reluctant to eat while here.
Is it okay for your pet to have treats or added canned food to their diet while boarding? If your pet has any food allergies or is on a special or prescription diet please select NO.
